OMG Survey was for real?

 Received an email like two months ago about taking a survey for my local dept of water. They supply the power for our cities, they offered me,  $150.00 off my electric and water bill, if I answered their survey.

Well, like most, get an email from folks asking me to fill out the survey. I never do. This survey looked like from the power company so I answered the survey. Couple days later, an email came saying I had won the survey enclosed below . Lol sometimes you have to take a "leap of faith".
